    End of life treatment refusal

    Hi All,
    Does anyone know how to obtain an end of life treatment refusal form in the UK as I believe you must have this in place while you are in a sound state of mind.

    Ask your GP for a DNR form - it will need to be signed by a healthcare professional. You will need to carry it with you and make your family aware that you have a DNR in place.

    Hi Sport, I have a DNR from my GP, that doesn’t cover refusal of food or medication, apparently there is another form you have to complete and your signature witnessed whilst you are in a position of sound mind and capable of making such a decision.
