I did mine through the government website. Some solicitors charge a lot to do something you can do yourself. I suppose it depends on what you wish to stipulate but mine was simple to do.Bulbar onset diagnosed Nov 21. No speech, feeding tube, limbs getting weaker but still just about mobile. -
We did ours ourself with a bit of advice from hospice staff. We did it at the same time as doing the ADRT and Respect forms.Carer for husband diagnosed with ALS April 2021. Hand onset. PEG fed, completely immobile, communicated with eye gaze
Sense of humour intact throughout.
