    I received the form a week later and my Nurse at the hospice signed and stamped it. Two weeks later after I posted it, a card can through with my personal number. Now, if I phone 195 and quote my number, I will not get charged for it.

    You might be elagable for this service, a form that a nurse can countersign. I can’t talk on the phone, but I am unable to hold and look through a directory so I will give it ago.

    Free enquiries service for those with disabilities

    There's a special 195 Directory Enquiries number available, without adverts, for those for whom using a phonebook may be difficult, eg, if you have memory loss, ME or a physical disability. Complete a form and get it countersigned by your GP or another professional familiar with your disability, then call 195 from any phone (landline or mobile) for more info. Textphone users can call 0800 838 363.

    If you're not able to read or hold a telephone directory due to illness or disability, you can apply for free 195 Directory Enquiries.
    Call 0800 587 0195 to request an application form. Your form will need to be countersigned by a professional who knows you i.e. your doctor, nurse or other medical practitioner.
