    Riluzole and ALT serum levels

    So second blood test and my alt serum level has gone up from 31 to 73, which is abnormal will this go back down? Am feeling a lot more tired hopefully will get me energy back up soon.

    Hi Steve. Mine went up to 58 and went down. I had that happen twice. Apparently its common until you've been on it sometime.

    You neurologist should guide you on it.

        Stevedp Even if your ALT level doesn't return to normal Steve, it doesn't automatically mean stopping Riluzole.

        There is quite some leeway before levels are concerning, up to 5x the upper normal level in fact (without liver impairment) and the ULN for men is 50 U/L.

        This is from the SPC for Riluzole:

        Riluzole should be discontinued if the ALT levels increase to 5 times the ULN. There is no experience with dose reduction or rechallenge in patients who have developed an increase of ALT to 5 times ULN. Readministration of riluzole to patients in this situation cannot be recommended.​
